Getting serious with year 2000: Bugs can sometimes be deadly
On July 26, 1985, a 40-year-old patient at the Ontario Cancer Foundation in Hamilton positioned herself under a Therac-25 for a blast of radiation to her cancerous cervix. When the operator activated the Therac-25, it shut down with an error message. The dosimetry display read "no dose." After resetting the machine, the operator tried again -- with the same result. The operator repeated the sequence three more times before calling a service technician. This wasn't unusual. The Therac-25s were notoriously temperamental. READ
